Returning home following hospitalization can be challenging. Planning for the unexpected often feels overwhelming. You can receive skilled nursing care in a hospital setting. We help you prepare to return home with the confidence and skills needed to resume your daily routine.

All skilled nursing patients stay in their own rooms, and enjoy the privacy of their own bathroom.

Recuperate comfortably

When possible, we encourage our residents to bring several sets of comfortable clothing. We won’t ask you to wear a ‘johnny’ unless your care requires it.

We offer free Wi-Fi and broad cable television choices.

Helping you plan for what comes next

Our experienced social work team will be meeting with you throughout your admission to offer assistance as you create your optimal discharge plan. MCH social workers will help you access local resources and are an excellent source of support for both you and your family.

Initiating a Referral

To recover close to home you and your family should discuss your needs with your physician or social worker/discharge planner. You will need a referral. Referral information can be faxed to: 603-924-4686.

Once we receive the referral:

  • Our treatment team will meet to discuss your needs and our ability to meet them.
  • If you meet the admission criteria, our assessment nurse will contact your insurance carrier to verify benefits, determine the need for additional information and obtain approval for a transfer to MCH.
  • Once approved, our nurse will notify the referring hospital’s discharge planner and work with our admitting office to coordinate your transfer/admission.
